In addition to the FAFSA, some colleges and universities have their own
financial aid application. This is called an institutional form. If the school
requires it, make sure it is completed by the deadline. Refer to the school's
website for information regarding their institutional form and deadline.
This form is a supplemental form and is not meant to be a substitute for the
FAFSA.
